Biotivia Friends & Family Longevity Program Details

As a member of Biotivia's Friends & Family Longevity program, you will earn one Friends & Family Longevity Point for every dollar you spend on eligible purchases made online at Biotivia.com or through Biotivia LLC phone orders. You will be able to track your Friends & Family Longevity Points online through your Friends & Family Longevity account. Your points are valid for 12 months from the time they are awarded and may be used towards the purchase on any Biotivia product sold on the Biotivia.com website.

Eligibility and Enrollment To become a member you must enroll first through the Biotivia website sign up form or by phoning (800) 458-0993. Once enrolled you will remain a member until cancelled by Biotivia or by electing to cancel your subscription.

Participation in the Friends & Family Longevity customer loyalty program is free of charge. Members agree to accept automatic recurring monthly purchases and delivery of at least one Biotivia product. Bonus points will be earned on the recurring purchase and all additional one time purchases. Friends & Family Longevity Points begin to accrue with the first purchase made after the enrollment process is complete. The member's unique identifier shall be his or her email address. In the event of a change of email address the member agrees to amend his or her profile to update it with the correct address.

Friends & Family Longevity Members who make a purchase at Biotivia.com must use the same email address used to enroll in the Friends & Family Longevity Program. Friends & Family Longevity Points may not be transferred to another account without the permission of Biotivia.

Biotivia LLC, Inc. ("Biotivia LLC," "we," or "us") reserves the right to monitor and review all customer transactions and activity associated with the Friends & Family Longevity program. We, in our sole discretion, may refuse or cancel any transaction or any account of any person we believe is engaged in activity that appears to be fraudulent, in violation of these Terms and Conditions, or inconsistent with the program's intended purposes. Biotivia LLC will have the sole right to make final decisions that interpret these Terms and Conditions and determine the earning and redemption of Friends & Family Longevity Points.

Employees of Biotivia LLC or any of its affiliates, including full time, part time and seasonal employees, are not eligible to participate in the program. This offer is void where prohibited by law. All federal, state and local laws and regulations apply. Program Benefits Customers must earn a minimum of 50 Bonus Points before they may apply these points towards a purchase.

From time to time Biotivia will elect to send members special pricing offers, free samples of new products, discounts on products and/or shipping charges and other rewards and benefits. These awards and benefits are at the sole discretion and volition of Biotivia.

Accruing Friends & Family Longevity Points valid for purchases made online at Biotivia.com or through Biotivia phone orders. You will earn one Friends & Family Longevity Point for every dollar you spend on Biotivia LLC products. You will not earn Friends & Family Longevity Points on shipping fees or taxes. Friends & Family Longevity Points are earned during an annual period beginning August through July.

Friends & Family Longevity Points expire at the end of the calendar year in which they were issued unless Biotivia elects at its sole discretion to extend the validity of such points.

Friends & Family Longevity Points will be posted to your account within 2 weeks from the time your order has been processed and shipped.

Friends & Family Longevity Points will be deducted from your account when a Biotivia LLC purchase is returned for credit to your credit card.

No points will be awarded for purchases before you enroll in the Friends & Family Longevity program. Program Changes and Termination Biotivia LLC, in its sole discretion, may change these Terms and Conditions, including the benefits and limitations of Friends & Family Longevity Points, or cancel the program entirely, at any time, and from time to time, without prior notice to you.

Changes in the program may adversely affect your ability to use Friends & Family Longevity Points in your account to make purchases at Biotivia LLC. Termination of the program will cause a forfeiture of all Friends & Family Longevity points and benefits.

We reserve the right to withhold Bonus Points and all other program benefits from purchasers who we determine to be acting as re-sellers or distributors of Biotivia LLC merchandise.

Your Privacy We take your privacy very seriously. Information you provide to us through your participation in the Friends & Family Longevity Program will be used by us in accordance with Biotivia LLC Privacy Policy, which can be accessed from a link at the bottom of our homepage.
